    Adverse event is an incident which results in harm to a patient. It is an event that results in unintended harm to the patient by the act of commission or omission rather than the underlying disease of the patient.

    While near miss are errors that occur but do not cause harm to the patient. Probably due to chance, prevention or mitigation

        Adverse events due to negligence: those that occurred due to care that falls below the standards expected of clinicians in the community.
        Two other terms define hazards to patients that do not result in harm:

        Near miss: an unsafe situation that is indistinguishable from a preventable adverse event except for the outcome. A patient is exposed to a hazardous situation, but does not experience harm either through luck or early detection.
        Error: a broader term referring to any act of commission (doing something wrong) or omission (failing to do the right thing) that exposes patients to a potentially hazardous situation.

    “adverse events” were defined as adverse outcomes that resulted from errors.3 Errors that did not result in patient harm, but could have, were categorized as near misses

    A Near Miss is an unplanned event that did not result in injury,
    illness, or damage – but had the potential to do so. Only a fortunate
    break in the chain of events prevented an injury, fatality or damage;
    in other words, a miss that was nonetheless very near.
    -OR
    A near miss is an unintentional incident that could have caused damage, injury or death but was narrowly avoided. In the context of safety, a near miss may be attributed to human error, or might be a result of faulty safety systems or processes in an organization

    An adverse event is “an injury that was caused by medical management (rather than the underlying disease) and that prolonged the hospitalization, produced a disability at the time of discharge, or both.” and they’re maybe avoidable and maybe not.
    A near miss is defined as “any event that could have had adverse consequences but did not and was indistinguishable from fully fledged adverse events in all but outcome.” In a near miss, an error was committed, but the patient did not experience clinical harm, either through early detection or sheer luck.
